We'll need these five simple things to implement your benefits for your small business today:

  1. Your preferred effective date

  2. Your company name and address 

  3. Your total number of eligible employees

  4. If applicable, a copy of the current carrier's group dental benefits summary/bill

  5. Your plan selections: You may select dental-only, vision-only, or dental and vision plans

     For dental plans: Minimum participation of two subscribers applies and 75% of eligible employees must reside in North Carolina 

     For vision plans: Minimum participation of two subscribers applies
